Story
We are the CAMHS- AMHS Transition Team and are part of the NHS Black Country Healthcare Foundation Trust. We currently support young adults who are transitioning between Child and Adolescent Mental Health Services and Adult Mental Health Services. There can be a “gap” between services, our team was commissioned to offer mental health support during this time of transition. Our Team are made up of Transition Workers, Mental Health and Wellbeing Practitioners and Peer Supporters. The localities we offer to support to are: Dudley, Sandwell, Walsall and Wolverhampton.
We now have the exciting opportunity to engage with our current young people in a community hub setting as well as expanding this reach to currently disadvantaged groups. As well as those who are transitioning from CAMHS-AMHS, we are now able to include those who are; neurodivergent, have left care settings, are young carers, are part of the LGBTQ+ community and many more. We aim to provide activities which will bring about a sense of connectedness, reflection and mindfulness in order to support their mental health needs.
We are in the very early stages of establishing our hubs, therefore donations would be greatly appreciated to kick start our community hubs.
Items that we're currently trying to buy for our hubs include:
Crafts: pens, paper, pencils, colouring pencils, card, paint, paint brushes, wool, crochet needles, knitting needles.
Games: Jenga, Bop It, Uno, Bananagrams, Dobble, Story Cubes.
