The Community NHS services in South Tees continually work to aim to to avoid hospital admissions for the patients in our region but to also support patients in the best possible way following a hospital stay. Most patients are at huge risk to readmit back into the hospital setting if not cared for in the best way. Services can be stretched and can sometimes be under intense pressure. 

I am in the process of setting up a newly developed service in order to try and improve the lives of those patients who are at risk of admission to Hospital and keep them safely supported in their place of residence. Ultimately, by improving their conditions at home, the hope is that they will never need to admit to Hospital in the first place. 

We will need some vital pieces of kit in order to provide the best possible care for these patients and I hope to raise to funds to start to purchase what we ultimately need.
