Derbyshire patients join with Ashgate Hospice and the frailty team to provide more care at home for the frail and elderly and in end of life situations.
What's the problem?
Most medical conditions that finally end our lives progress gradually with crises at certain times.
The commonest cause of death in the UK is now dementia but other ‘long term conditions’ are close behind eg. heart disease, cancers, stroke.
Most of us would like to have most of our care at home and be given the best chance to die in our place of choice.We would like to be able to say how we would like our care to be managed and hope that our wishes will be followed.
Sadly, during a crisis moment there are often not enough community resources available to support someone safely at home and the ambulance is called and hospital admission follows.
Hospital admission at these times often brings major problems such as rapidly reducing mobility and reducing chances of being able to return to our homes.
Many people do not die in their preferred place.
Let's change things
Ashgate Hospice and the frailty team have skilled teams which visit people at home and offer expert help. They listen to the patient’s preferences for their care and try to ensure these choices are followed.
