The Bradford Teaching Hospitals Children's Community Team is a multi disciplinary service comprising Clinical Nurse Specialists, Physiotherapists, Children's Nurses, Dieticians, Pharmacists, Health Care Support Workers, Medical and Admin staff. 

Our services cover the Bradford and Airedale district's and we provide a wide spectrum of care to families from birth through to adulthood.  Our aims are to facilitate safe discharge from hospital for children to their family home.  To expertly support children and families through their conditions and maintain a level of health that avoids re-admission to hospital. We also provide respite care to families of children with complex needs in addition to supporting families of children with life limiting conditions.

Whilst the care we provide is funded by the NHS we can make our services go above and beyond with the support of the Bradford Hospitals Charity.  This support through your kind donations helps us to support families in their time of need.  It helps us to go the extra mile for the families of Bradford and Airedale.
