Story

Beyond Horizon

Our ‘Beyond Horizon’ transition pilot project aims to help teenagers and young adults with life limiting conditions to move from children’s services into adult services whilst also supporting their parents at what can be a tremendously stressful and worrying time. These young people and their families find themselves in the difficult position of being too old for children’s hospice services, but still needing support to make the transition to adult services.

However, developing these new and vital services presents many challenges, especially financially. That is why we hope you will consider making a donation today to help us raise the funds needed to carry on our work. Please make a donation of £30 or whatever you can afford.
