In honour of dad, we have opened this giving page to accept donations to the Calderdale & Huddersfield NHS Trust Palliative Care Team who gave dad such fantastic, compassionate and attentive care in his final days at home with us.

The fund is managed by the team and is used for a variety of things ranging from additional equipment, to literature and resources for families, even essential household items for families caring for someone at home who may be struggling. It is completely separate from the teams NHS funding and is used at their discretion without the need for debate and bureaucracy when it is needed.

The whole team were completely amazing when dad came home and couldn't do enough for him and our family in his final weeks. Their support and compassion continued even after dad had passed away and as a family we will be eternally grateful to all of them for everything they have done 

At this most difficult of times, it is a huge comfort to know that dad had such brilliant care in his final days and we just wanted to find some way of saying thank you to those who provided it.
