VNA Health Care is a humanitarian, not-for-profit organization that provides comprehensive primary care and community health services for people living in suburban Chicago and beyond. VNA strives to improve health equity and reduce health disparities with high quality, accessible, and compassionate care delivered in VNA health centers, community settings and patient homes. Federally funded community health centers (CHCs) provide affordable healthcare options to more than 28 million people in towns and cities across the U.S. The clinics are designed to improve access to primary care services regardless of whether patients have insurance, speak languages other than English, or live in remote areas.
VNA continues to fulfill that mission with Illinois' newest residents.
A VNA hospital liaison recently assisted a new patient who was a refugee from Venezuela. The patient delivered her baby with a VNA OB provider serving as that days backup. Because the patient didnt have a pediatrician, the hospital nurses asked the VNA hospital liaison to meet with the mother to assist with her follow-up care. She was able to schedule a wound check and follow-up Baby appointments with a VNA pediatrician, as well as connect the mother with VNAs WIC and Healthy Families teams.
VNA provided a crucial safety net to provide this burgeoning family with the resources needed for a fresh start in their new home. Community Health Centers such as VNA play a crucial role in building a healthier community for all.
