The SWAN model will vastly improve the experience for the patient and their loved ones at an extremely difficult time. I want to ensure that nothing is too much trouble; I want them to feel that there is compassion and understanding from everybody from the cleaner to the consultant. I want information to be given freely and communicated to families and our patients in a tailored way. Most importantly, our SWAN model will allow us to support our patients and their families and allow them to be involved in decisions being made regarding their care and their wishes. Your support will help us to transform our end of life care; to make a positive difference to our most vulnerable patients and their families, and its just incredible. Thank you.

£15,000 , would fund Memory Boxes across all out hospital sites and would support over 2,000 patients in just one year.
£50,000 , would fund a SWAN Suite within hospital grounds, a beautiful, peaceful and private area where families and loved ones can be given time and compassionate care by our dedicated teams. Equipped with comfortable furniture, refreshments and thoughtful artwork it will also host a book of remembrance, toys and books for children visiting SWAN Suites and resources for supporting children and families through bereavement.
